流感疫苗接種活動~疫起提升保護力吧!!(光復校區)
本活動為自費四價流感疫苗注射活動
- 日期 : 2024/09/26 星期四
- 時間: 09 :00 ~ 11 : 30 (依報名梯次報到→填疫苗同意書→醫師評估→安排疫苗注射)
- 地點: 大禮堂二樓-會議室(交大校區) 請由靠近郵局側樓梯 上二樓
- 疫苗名稱: 四價 菲流達Vaxigrip Tetra 流感疫苗(法國原廠) – 850元/劑 (請自備零錢)
- 對象: 18歲以上 本校教職員工生
- 報名網址: 9/18 星期三 中午12:30開放報名，請至校園單一入口/陽明交通大學/全部或校園服務或學務/健康自主管理系統/活動預約 (額滿截止)
◊如需取消注射，請務必於活動前三天來電告知，避免疫苗浪費。

This event is a self-paid quadrivalent influenza vaccine injection event.
- Date: Thursday, September 26, 2024
- Time: 09:00 to 11:30
- (Participants should arrive according to their registration times→ Fill out the vaccine consent form → Medical evaluation by a physician → Arrange for vaccine injection)
- Location: Second floor of the Grand Auditorium - Meeting Room ( Guangfu Campus ). Please use the stairs located near the post office to access the second floor.
- Vaccine Name: Vaxigrip Tetra Influenza Vaccine(France) – NT$850 per dose (Please prepare exact change)
- Eligibility: Faculty and staff and stundent
- Registration Website: Registration will open at 12:30 PM on September 18th. Please visit the NYCU Portal -> NYCU Campus -> Health Management Systems -> Event Reservation -> Event Application. (Registration will be closed when quota is full).
